With Know My Story, you will capture the experiences, preferences, strengths, and dreams that make your loved one unique, creating a resource that can guide future support and decision-making for years to come.
When support staff change, services transition, or family members are no longer able to speak alongside their loved one, Know My Story helps ensure their voice, preferences, and life experiences are not lost.

We believe life planning helps individuals and families move through life's transitions with greater confidence and peace of mind. It supports self-advocacy, promotes independence, reduces family stress, and provides a roadmap for future decisions—helping ensure that the person's voice, preferences, and goals remain at the center of the journey.
It greatly contributes to enhancing quality of life, strengthens self-advocacy, supports greater independence, reduces family stress, as well as bringing clarity and direction for what comes next.
Additional support options are currently in development to help individuals, families, and professionals make the most of the Know My Story workbook.
This will include guided consultation and facilitated life-planning support for those who want help gathering information, completing sections of the workbook, or developing a more comprehensive written plan.

Know My Story is about ensuring the voices of people with disabilities are heard, respected and valued. "Know My Story" is hopefully the first of many resources and tools that ensure people know their (people with disabilities) story and how to use that knowledge to provide the best support, advocacy, and cultivate connection in care.
My name is Lacey Freeman (Eaton) and I am a Licensed Professional Counselor and a two-time graduate of Texas Wesleyan University with a BA in Psychology and a Master of Arts in Professional Counseling and a Positive Psychology Coach.
Since 2004, I’ve been walking alongside family caregivers and professionals supporting people with physical, mental, and intellectual differences and disabilities. I’ve specialized in disability services since 2008, working across local, state, and federal programs to expand access, provide crisis prevention and intervention, and provide training for professionals in mental health and IDD, person-centered, and trauma-informed care.
Through my practice, I’ve offered counseling for people with IDD, risk management training for Direct Support Professionals (DSPs), professional development, leadership development, and personalized consultation for families, educators, and care teams.
Over the years, I’ve heard one question echo from countless families: “What will happen to my loved one when I’m no longer here?” That question, along with the frustration of deficit-based systems, deeply shaped my path.
This book grew out of those conversations: born from both professional experience and a personal commitment to helping families plan for the future with clarity, compassion, and strength. It’s a passion project, but more than that, it reflects the values I live by in every aspect of my work.
